Piper Alpha, part 1
On July 6 1988, Piper Alpha was hit by an explosion that caused fires killing 167 people; 61 workers survived. In this act, the eye-opening reasons behind the explosion will be shown, which raises the question: why did this disaster happen with many warning signs in advance?

Remember Charlie
In 1980, Charles T. Morecraft was an employee of Exxon Corporation. He was a good employee ­ both in the eyes of his union and in the eyes of his management, but otherwise he was unremarkable. He was just like hundreds of thousands of other blue-collar workers. He never considered himself exceptional. He was "just one of the boys".

Charlie had more than 15 years experience on the job, and he knew it well. He knew all the rules. He knew all the safety regulations . . . and he knew all the shortcuts around them too. Yes, Charlie was just like all the other seasoned workers at the refinery; he was certain that nothing could go wrong when he was in control. And he was right ….. Except for this one time.

How structures create culture
Organisations sometimes set out to create cultures of safety using the hearts and minds approach, that is, running workshops for employees encouraging them to think differently about safety.
This presentation argues that the real key to developing a culture of safety is having the right organisational structure. This means ensuring that the people with special responsibility for safety are sufficiently senior and sufficiently powerful to be able to discharge their responsibilities effectively.  This requires a relatively centralised organisation. This is illustrated using the Columbia space shuttle accident.  The presentation includes a case study of BP, looking at its organisational structure before and after the Deepwater Horizon accident.

Michael Borrell will take us on Total’s Safety journey - moving from unsatisfying results to best in class from the late 90s until today. He will provide insights on Total’s unique HSE Management System Framework and zoom in on what’s at stake with major risks. Michael Borrell will touch upon Total’s efficient management and analysis of asset integrity and close his presentation with a reflection on the intrinsic value of engaging employees and contractors in the safety journey.

The challenge of the unexpected
Better, Leaner, Cheaper … and SAFER! The title of the conference sums up very well the challenge facing the oil & gas industry, like many others. This challenge obviously raises specific questions about the latter item: safety. It generally follows from the debate that yes, there may be safety risks to making too much savings, or putting too much pressure on operations, but ultimately, all things considered, the qualities necessary for superior operational performance - such as good procedures, good equipment, good leadership, good communication, good synergy, are also the ones that guarantee safety. There is truth in this vision, but perhaps it is incomplete. In his presentation, Jean Pariés will show that optimization strategies - based on the anticipation and standardization of “one best way” responses, also have a perverse effect: by reducing the diversity and variability of systems, they reduce what is called their resilience: the ability to handle the unexpected. He will also suggest ways to preserve this capacity, because it is more and more necessary in a world paradoxically at once more and more regulated and unpredictable. Indeed, day after day, the news from the world confirms the growth of change, turbulence and complexity, the reign of surprise, and the need for adaptability and resilience.

As a former oil business executive with now almost three years at the side-lines of the industry, Jakob Thomasen will reflect on what is next in future safety of the oil industry. Jakob’s views will be fueled by his 32 years in the oil industry as well as solid engagement in other businesses over the last few years.

Perhaps you want to become more effective and faster at work, you want to become better to remember or motivate yourself, but how do you accelerate your brain? Troels W. Kjær has experimented with brain training and will in the presentation share his best pieces of advice and provide specific tools to optimize one´s awareness, memory and learning. You will get more out of your life, and, perhaps, you will become better to remember your wife´s birthday and your daily errands. Troels W.
